How to Apply?
Application form along with attested copies of supporting documents/certificates and Demand Draft of Rs.200/- payable in favour of Chief Medical Officer Rajouri, which must reach in the Office of the Chief Medical Officer Rajouri by hand against proper receipt within 21 days from date of publication of this notice.
Documents to be enclosed with the application form:
- Date of Birth Certificate
- Marks Certificate of 10th
- Marks Certificate of 10+2
- Basic/ essential qualification certificates.
- Degree/ Diploma of requisite Qualification.
- Marks sheet of all years/one consolidated marks sheet indicating marks of all the years of the Degree /Diploma.
- Domicile certificate issued by Tehsildar concerned.
- 2 Passport size Photographs.
